Private Duty Nurse; RN​/LPN

Job Description

Night Shift – Open. Provides services to clients in accordance with the state Nurse Practice Act, Agency policy, and accepted professional standards of practice. The Registered Nurse (RN) provides nursing services utilizing a comprehensive base of nursing theory under the direction of a medically approved care plan.

Reports to:

Clinical Supervisor
Provides supervision to:
None – No supervision responsibilities.

• Graduate of an accredited school of professional nursing (BSN preferred).
• Current license as a Registered Nurse in the state of operation.
• Demonstrated strong communication and interpersonal skills.
• Current CPR certification.
• Licensed driver with automobile insured in accordance with state and/or agency requirements.

• Assume responsibility and accountability for the practice of professional nursing in accordance with the state Nurse Practice Act and standards for home health nursing.
• Perform physical assessments of clients, and may complete comprehensive assessments for client admissions and reassessments at the discretion of the Clinical Supervisor.
• Supervise Licensed Practical/Vocational Nurses (LPN/LVNs) and/or Home Health Aides (HHAs) at the discretion of the Clinical Supervisor.
• Report changes in client condition promptly to the medical practitioner overseeing care and to other members of the client’s care team.
• Participate in the development of the client care plan.
• Consistently demonstrate competency with technical nursing skills according to personal and legal scope of practice.
• Practice nursing with respect for individual, cultural, and spiritual differences.
• Follow agency policies and procedures as well as payor and accreditation requirements.
• Initiate actions to improve the quality of client care and to prevent negative outcomes. Continuously promote safety and infection prevention and control practices.
• Maintain confidentiality in all aspects of the job.
• Participate in ongoing, professional self‑development.
• Delegate tasks to Licensed Practical/Vocational Nurses (LPN/LVNs) and/or Home Health Aides (HHAs) based on their legal scope of practice and where circumstances require.
• Perform other duties as deemed necessary.
