Job Overview
Reserve Care, formerly Hospice of the Western Reserve, is seeking a Registered Nurse to provide skilled nursing services and personal care to terminally ill patients in collaboration with a transdisciplinary team across various settings.
Responsibilities- Practice in accordance with HWR clinical standards, policies, and procedures.
- Assess the physical, psychosocial, and spiritual needs of patients and their families or caregivers.
- Provide intermittent skilled nursing services under the patient’s physician’s direction.
- Monitor and evaluate pain and symptoms, offering information and assistance to manage these issues.
- Serve as a liaison to the patient’s physicians when needed, providing relevant information regarding patient needs.
- Offer emotional support to patients and their families or caregivers.
- Refer patients and/or families to other services as appropriate and follow up.
- Provide health teaching instruction to patients and families.
- Maintain and promptly complete all necessary plans and reports related to hospice care.
- Participate in care conferences and bereavement follow-up.
- Attend case reviews, quality improvement activities, and other meetings as assigned.
- Plan and clinically supervise activities of HNA related to patient care.
- Participate in the on‑call rotation as scheduled.
- Collaborate with team members on discharge planning.
- Communicate by phone or in writing with case managers as determined case‑by‑case.
- Assist the team with coordinating the Plan of Care, including coordination with volunteer staff.
- Identify opportunities for volunteers to serve patients and families and coordinate communication with volunteer staff.
- Demonstrate positive customer‑service responsiveness according to agency standards.
- Be flexible in meeting patient/family needs that require variable hours.
- Maintain standard and transmission‑based precautions.
- Complete annual core competency requirements.
- Participate in public relations, community outreach, and promotion of the hospice.
- Assist with orientation of new employees.
- Assist with in‑service programs as requested.
- Serve on committees as appropriate or assigned.
- Assist with education/precepting of students.
- Licensed RN in the State of Ohio.
- Experience in hospice care, home health, oncology, and/or pain and symptom management preferred.
- Current CPR certification.
- Valid driver’s license, good driving record, and auto accident insurance at a level acceptable to HWR (not required for Hospice House staff).
- Hospice and Palliative Care Certification desired; certification required within the first two years of hire.

- High level of integrity, ethics, and professionalism.
