- Ensure all activities align with the vision of Elara Caring’s leadership and the Home Health team.
- Maintain patient and staff privacy and confidentiality pursuant to HIPAA.
- Assess assigned case load of home‑health care patients and families to identify physical, psychosocial, and environmental needs.
- Assume primary responsibility for caseload, including assessment, planning, coordination, implementation, and evaluation of patient care plans.
- Obtain data on physical, psychological, social, and spiritual factors that influence patient/family/caregiver health status and incorporate that data into care plans.
- Make initial and follow‑up evaluations of patient nursing needs during each visit, adjusting visit frequency as required.
- Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physicians, facilities, and other personnel involved in patient care.
- Revise the plan of care in response to identified issues and notify the Clinical Supervisor and team members.
- Report to the Clinical Manager or Clinical Supervisor.
- Current, unrestricted state RN licensee.
- At least 1 year of clinical nursing experience.
- Able and willing to travel within the branch/office coverage area; must be able and willing to travel 50% of the time.
- Ability to frequently lift, push, pull, and support up to 50 pounds.
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Associate’s Degree or Bachelor’s Degree in Nursing preferred.
- Previous Home Health or Hospice experience preferred.
- Previous experience with Home Care Home Base preferred.
