Position Summary

The Registered Nurse (RN) provides nursing care to terminally ill Hospice patients as needed. The RN aids in understanding for the family in the home care situation and in times of bereavement, and works as a member of the Hospice team to provide Hospice care.

Required Experience
  • Graduate from an accredited school of nursing
  • Currently licensed as a registered nurse to practice in the state
  • Minimum 2 years of experience as a registered nurse, preferred
  • Active patient contact within past three years, preferred
  • Ability to work independently, make accurate, and at times, quick judgments
  • Ability to supervise others appropriately
  • Ability to respond appropriately to crisis outside of a hospital setting
  • Acceptance of and adaptability to different social, racial, cultural and religious modes
Accountability
  • Regularly assess and re-assess the nursing needs of Hospice patients
  • Provide dietary counseling
  • Provide Hospice nursing services, treatments and preventive procedures
  • Initiate nursing procedures appropriate for the patient’s Hospice care and safety
  • Observe signs and symptoms and report to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition
  • Teach, supervise and counsel the Hospice patient and family members about providing care for the patient
  • Supervise and train other nursing service personnel
  • Develop and re-evaluate the patient/family care plan in conjunction with IDG to meet needs and maintain continuity of care
  • Perform specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders
  • Attend team conferences
  • Maintain records as required by Hospice
  • Follow policies and procedures of Hospice; observe confidentiality and safeguard all patient-related information in compliance with HIPAA regulations
  • Communicate to the supervisor if unable to meet a patient’s need or perform a procedure
  • Participate in on-call system and be responsible for providing on-call coverage when unavailable for assigned duties
  • Work with interdisciplinary group concept of patient care
  • Coordinate the implementation of the plan of care for patients residing in SNF, NF, ICF or MR
  • Organize work schedule and utilize time management to attend all required meetings
  • Comply with agency infection control policies and protocols
  • Assist with orientation, teaching and training as requested
  • Other duties as assigned by Director
General Working Conditions
  • Employee stands for long periods and may sit, walk, use hands to handle keyboard, telephone, paper, files, and other equipment and objects
  • Employee may reach with hands and arms, review detailed documents and read computer screens
  • Employee may occasionally lift or move up to 25 pounds
  • Noise level moderate; occasional travel to different locations may be required
