Hospice Admission Nurse

Overview

• Promote Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
• Assess assigned visits of hospice patients to identify physical, psychosocial, and environmental needs as documented in the clinical record and admission paperwork.
• Participate in the implementation of the plan of care for an assigned caseload to ensure quality, continuity of care, and achievement of patient outcomes.
• Make the initial evaluation and re-evaluate the patient’s nursing needs during each visit.
• Communicate significant findings, problems, and changes in condition or environment to the Patient Care Manager, the physician, and/or other personnel involved with patient care within 24 hours.
• Report unsafe conditions to the appropriate Clinical Supervisor and physician in a timely manner.
• Identify deviations from normal nursing and patient condition status, revise the plan of care and initiate appropriate actions based on the deviation.
• Perform other duties/projects as assigned.

• Graduate of an accredited nursing program.
• Current, unrestricted state RN licensee.
• One-year experience in a clinical care setting as a nurse.
• Able and willing to travel within branch/office coverage area and 50% of the time.
• Must have a dependable vehicle, valid driver’s license and current auto insurance in accordance with state laws.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Associate’s Degree or Bachelor’s Degree in Nursing preferred.
• Previous Home Health or Hospice Experience preferred.
• Previous experience with Homecare Home Base  preferred.
