  • $125,000 to $170,000 Annual Income
  • $105,000 to $125,000 Base Salary

They currently boast a team of more than 30 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider's office. Their services include, among others, primary care, psychiatric care, post-acute care, and transitional care.

POSITION

DESCRIPTION

The nurse practitioner will serve as the primary care provider for geriatric patients located within a single skilled nursing facility (SNF) in Mentor.

Providers see a variable number of patients per day, dependent upon patient needs.

Specific duties of the role include, but are not limited to:

  • conduct health assessments to identify patient needs
  • develop personalized care plans
  • regularly evaluate patient conditions and responses to treatment
  • adjust treatment plans as necessary
  • perform minor procedures and wound care as needed
  • educate patients and families on health management, disease prevention, etc.
  • provide guidance on lifestyle modifications and self-care practices
  • work with physicians, nurses, and other professionals to coordinate care

The practice uses a mobile-based EHR with AI functionality to reduce administrative time.

On-call is phone triage only and providers rotate the responsibility evenly, with some flexibility in scheduling available.

REQUIREMENTS
  • Ohio NP license
  • DEA
  • new grads with backgrounds in home health, LTC, etc. are encouraged to apply
  • no experience required
