Job Description:

Hybrid position: 3 days in office and 2 days remote.

As a Transitional Liaison LPN/LVN you'll contribute to our success in the following ways:

* Identify patients who may qualify for hospice care.

* Follow the hospice referral, eligibility, and documentation process.

* Assess patients' physical, emotional, social, and spiritual needs.

* Work with care teams to coordinate patient transitions and services.

* Meet with patients and caregivers to discuss care goals and service options.

* Explain hospice services so patients and families can make informed decisions.

* Educate staff and referral partners on hospice eligibility and the transition process.

* Support the hospice team by coordinating and assisting with admission visits.

What is Required?

* Current state license as Licensed Practical Nurse/Licensed Vocational Nurse

* Graduate of an accredited LVN/LPN Program

* 3+ years' experience in a clinical care setting

* Experience with in Home Health or Hospice is preferred
