Assistant Director Of Nursing (ADON) / Infection Preventionist

Care Core at Mentor is seeking an experienced and motivated Registered Nurse (RN) to join our leadership team as our Assistant Director of Nursing (ADON) / Infection Preventionist.

This is a great opportunity for an RN with strong long-term care experience who enjoys being hands-on, supporting clinical staff, and maintaining high standards for infection prevention, resident care, and regulatory compliance.

• Assist the Director of Nursing with day-to-day clinical operations
• Serve as the facility's Infection Preventionist
• Monitor, track, and investigate infections and infection trends
• Maintain infection prevention and control policies, procedures, and documentation
• Provide infection control education and training to staff
• Assist with survey readiness and regulatory compliance
• Support nursing staff through education, coaching, and clinical oversight
• Participate in audits, quality improvement initiatives, and resident care reviews
• Assist with staffing, employee performance, and clinical concerns as needed
• Provide leadership coverage and support to the DON and nursing department

• Active Ohio RN license required
• Previous long-term care/skilled nursing experience required
• Infection prevention/control experience strongly preferred
• Previous nursing leadership or supervisory experience preferred
• Strong knowledge of state and federal LTC regulations
• PCC/Point Click Care  experience preferred
• Strong communication, organization, and leadership skills
• Must be comfortable working collaboratively and providing hands-on support when needed
