St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to guide patients and families in their homes. You will collaborate with a care team to create personalized plans, lead meaningful end-of-life conversations, and ensure symptom management with empathy and clinical excellence.
You should have an RN license with a degree in nursing and at least 1 year of acute care experience; hospice/home health experience is preferred. Full-time with on-call expectations, located in Wisconsin.
