RN - Residential (Lawlis Family Hospice

Overview

Horizon is a home care and hospice provider dedicated to delivering compassionate care to patients and families.

• Perform referral evaluations / admission assessments of patients and families seeking hospice services.
• Perform physical, psychosocial and spiritual assessments utilizing non‑verbal cues, probes or modified questions to obtain comprehensive assessments.
• Communicate patient and family expectations of care, goals and learning needs to all members of the health care team through appropriate documentation.
• Ensure all documentation is clear, legible, concise, accurate, and timely, meeting department standards.
• Provide case management functions as part of the IDG to an assigned caseload.
• Provide services at quality levels consistent with professional standards of practice, approaching patients and families with care, concern and compassion.

• Degree from an accredited school of nursing (BSN preferred).
• Current WI RN license.
• 1‑2 years of hospice, oncology or related experience.
• Experience in caring for the terminally ill and knowledge of hospice care.

Part‑time (24 hours per week). Varying hours from 3 PM to 11:30 PM, including every other weekend and holiday rotation.

Hourly, non‑exempt.
