• Evaluate patients and develop individualized occupational therapy treatment plans that promote independence
• Implement therapeutic interventions to improve ADLs and functional mobility
• Collaborate with an interdisciplinary rehab team to support safe, effective patient care
• Educate patients and caregivers on safety, exercises, and adaptive techniques to empower recovery
• Document clinical findings and progress accurately and timely to support continuity of care

• Active California Occupational Therapist (OT) license required
• SNF or rehab experience preferred
• Strong clinical reasoning and communication skills to drive patient-centered care
• Ability to work independently and as part of a collaborative team
• Comfortable with timely and accurate documentation practices

• Patient evaluation and treatment planning
• Clinical reasoning and evidence-based interventions
• ADLs and functional mobility training
• Interdisciplinary collaboration and teamwork
• Patient and caregiver education
• Accurate clinical documentation
