Job Title: Per Diem LVN / LPT

Division/Program: Merced Crisis Residential Unit (Adult Inpatient Program)

Starting Compensation: 32.00 - 34.00 USD Per Hour

Working Hours/Shift: Per Diem coverage needed for 12-hour AM and/or NOC shifts (06:00–18:30 / 18:00–06:30). Coverage is as needed, based on facility needs and the candidate’s availability. NOC shift includes a $2 differential.

• High School diploma or equivalent (e.g. GED, HiSET, etc.) required
• Associate degree in nursing or social science field preferred.
• One (1) year nursing experience required, preferably treating the mentally ill in an inpatient setting. (Clinical nursing school rotations may be substituted for a portion of work experience.)
• One (1) year medication administration experience preferred.
• Valid California LVN or LPT license required.

• Provides direct nursing care and supervises same provided by all nursing staff on the shift worked.
• Visually and audibly observes, documents in writing, and reports orally or in writing the physical or mental condition to the Program Manager and the attending physician or nurse practitioner.
• Orders, prepares, administers, and documents the administration of medication for clients according to physicians’ or nurse practitioners’ orders. Confirms delivery of all ordered medication from pharmacy on shift worked.
• Responsible for writing a weekly summary on clients as assigned.
• Monitors and maintains appropriate nursing documentation in the medical record for shift worked.
• Prepares, presents, and coordinates or supervises a verbal or written change of shift report.
