For this role, you need to be a Nurse Practitioner who sees the value of hospice care in alleviating pain and suffering. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. We expect you to be knowledgeable in caring for the terminally ill, providing expert symptom management and providing emotional support. The goal is to help patients live their final days with dignity and comfort.

Skills
Experience in hospice or home health preferred
Knowledge of nursing care processes and medication guidelines/side-effects
Committed to safety precautions and high ethical standards
Proficient in reading/writing English, bilingual a plus (Spanish preferred)
Sound judgement and attention to detail
Excellent communication skills and a compassionate nature
Cool-tempered and patient
Emotional/mental stability and stamina
Requirements
Completion of Masters Program in nursing. Current NP Licensure in the state of CA.1 year Nurse Practitioner experience required
Current DEA license
Hospice & palliative experience preferred
Current CPR certification required
Must have reliable transportation, valid drivers license and automobile insurance
Responsibilities
Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
Collaborate with physicians and the Interdisciplinary Team to formulate and review care plans
Manages health conditions and terminal illnesses by developing an individualized plan of care, prescribing medications and / or treatments, obtaining consultations or making referrals and coordinating hospice care services.
Assists the patient and family/caregiver and other team members in providing continuity of care Alleviate symptoms to the patient's desired level of comfort
Notify a doctor for a patient’s condition when deemed necessary
Maintain accurate records
Help relatives cope with the upcoming loss and provide post-mortem support
