Position Overview

Registered Nurse – Los Banos, Dos Palos, and surrounding areas.

Hourly and Paid Per Visit (PPV) hybrid model (negotiable). Salary option available. Opportunities to accumulate overtime.

Acute care experience preferred.

Responsibilities
  • Complete an initial, comprehensive, ongoing assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illnesses.
  • Provide professional nursing care utilizing all elements of the nursing process and in accordance with the state Nurse Practice Act.
  • Assess and evaluate the patient’s status by:
    • Writing and initiating a plan of care.
    • Regularly re‑evaluating patient and family/caregiver needs.
    • Participating in revising the plan of care as necessary.
  • Initiate the plan of care and make necessary revisions as patient status and needs change.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, involving the patient and family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician.
  • Counsel the patient and family in meeting nursing and related needs.
  • Provide health care instructions to the patient as appropriate based on assessment and plan.
  • Assist the patient with activities of daily living and facilitate the patient’s efforts toward self‑sufficiency and optional comfort care.
  • Act as case manager when assigned by the clinical supervisor and coordinate care for the assigned caseload.
  • Communication
  • Complete, maintain and submit accurate clinical notes regarding patient condition and care delivered.
  • Communicate with the physician regarding patient needs and report changes in condition; obtain and receive physicians’ orders as required.
  • Coordinate the care plan with community health personnel.
  • Teach patient and family/caregiver self‑care techniques as appropriate; provide medication, diet, and other instructions as ordered by the physician.
  • Maintain a safe environment for the patient.
  • Assist patient, family/caregiver, and other team members in providing continuity of care.
  • Work with the family/caregiver and interdisciplinary group to meet emotional needs of patient and family.
  • Attend interdisciplinary group meetings.
  • Additional Duties
  • Participate in on‑call duties as defined by the policy.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Maintain and upgrade professional knowledge and practice skills through continuing education and in‑service classes.
  • Fulfill accepted case assignments.
  • Actively participate in quality assessment and performance improvement teams.
  • Qualifications
  • Graduate of a National League of Nursing accredited school of nursing.
  • Current license in California and current CPR certification.
  • Registered nurses must have at least: one year of experience as a professional nurse within the last three years OR a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license. Minimum two years’ experience, at least one year in public health, home care, or hospice nursing, is preferred.
  • Must be a licensed driver with an insured vehicle in good working order.
  • Management experience not required; responsible for supervising hospice aides.
  • Self‑directed and able to work with minimal supervision.
