About the RoleDuring a visit lasting up to one hour, providers review and observe a member’s health, medical history, medication adherence, social environment, and other risks. They conduct adult/geriatric assessments, administer point-of-care testing, collaborate with physicians and the care team, and provide services at members' homes or skilled nursing facilities.
Key Responsibilities- Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation per Matrix guidelines and protocols.
- Administer and order point-of-care and laboratory testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel.
- Work with the Primary Care Physician on patient education and follow-up as necessary.
- Provide services at the member’s place of residence (home or skilled nursing facility).
- Flexible scheduling: assessments Sunday-Saturday from 8 am-7 pm; work during day, evening, or weekend based on availability.
- Competitive pay: earn income based on the number of assessments completed; monthly average 20-100 visits yields $1,800-$11,500 based on $90-$115 per assessment.

- Comprehensive onboarding training, preceptor, 1:1 feedback, and free continuing education.
- Support for obtaining additional state licensure or credentialing if business need exists.
- Full-time opportunities for PRN providers with sign-on bonus.
- Master’s Degree or equivalent experience with satisfactory completion of NP licensure.
- Current NP licensure in the state of practice, including prescription authority or ability to obtain it.
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- 1 year of NP experience preferred; new graduates encouraged to apply.
- Valid state driver’s license for the state of residence.
- Automobile insurance coverage required for the state of residence (or ability to obtain).
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
- Bodily Injury: $100,000 per person/$300,000 per incident.
- Property Damage: $50,000.
- Collision and comprehensive deductibles: $1,000 or less.
