Position OverviewUnder the direction of the Clinical Manager, the Mental Health Specialist delivers intensive community‑based services to adults enrolled in the Full Service Partnership (FSP) program. The role utilizes Assertive Community Treatment (ACT) and Forensic Assertive Community Treatment (FACT) approaches to provide case management, rehabilitation, crisis intervention, care coordination, and supportive services in clients' homes and community settings. The specialist works as part of a multidisciplinary team to support individuals in achieving recovery, stability, and long‑term success while reducing the need for hospitalization, residential placement, or justice system involvement.
EssentialJob Functions and Responsibilities
- Deliver high‑quality case management to help clients access medical, educational, social, pre‑vocational, and rehabilitative services, ensuring effective communication, coordination, referrals, linkage, and progress monitoring.
- Provide rehabilitative services to enhance clients' functional, daily living, and social skills, including medication education.
- Lead services primarily in the field, offering hands‑on, personalized support and responsive, on‑call assistance to prevent crises.
- Collaborate with team and agency partners to ensure timely service delivery, measurable progress, and positive outcomes.
- Apply culturally appropriate, evidence‑based practices to engage clients effectively and deliver impactful services with humility and respect.
- Facilitate skill‑building groups to enhance clients' self‑sufficiency and life skills.
- Advocate for clients by navigating and securing critical resources on their behalf.
- Provide linkage and brokerage to specialized services.
- Conduct regular client meetings to assess progress and align with treatment goals.
- Engage clients to establish trusting, collaborative relationships focused on securing and sustaining competitive employment.
- Assist clients in understanding benefits (e.g., SSI, Medicaid) and how employment may affect them.
- Offer community outreach services to support clients who may appear to disengage from services.
- Comply with all agency policies, procedures, and HIPAA guidelines.
- Submit accurate records of service information and tracking consistent with program policies.
- Maintain records, prepare reports, and perform other administrative duties as required.
- Adhere to appropriate charting, documentation requirements, and agency regulations in a timely manner.
- Provide input and feedback on related projects and programs.
- Support agency‑wide Quality Improvement efforts.
- Adhere to ACT and FACT modes of service.
- Provide individual and group education appropriate to clients’ age, condition, culture, and learning readiness.
- Establish and maintain effective relationships with recipients of behavioral health services.
- Implement evidence‑based interventions that support the treatment plan and ACT/FACT models.
- Recognize and respond to symptoms of mental disorders and substance abuse.
- Modify and adapt evidence‑based approaches when a clear evidence base is lacking.
- Perform other related duties as assigned, attend trainings, and participate in relevant events.
