• Experience as a social worker within a nursing home or rehabilitation center strongly preferred.
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
• CT Social Work License (Required).
• Admission: 1 year (Preferred).
• Long term care: 2 years (Preferred).

• Plan, organize, develop and implement all required social services to meet the needs of residents in accordance with federal, state and local guidelines that govern skilled nursing facilities.
• Perform all documentation requirements for social service documentation in a timely fashion.
• Complete all admission assessments and evaluations.
• Work with the interdisciplinary team to create a plan of care for residents.
• Work with and provide support to residents, families and staff.
• Attend weekly Care Plan and Medicare meetings.
• Receive and attempt to resolve resident grievances.
• Coordinate resident discharge planning with all relevant departments and assist with scheduling outside services.
• Participate in monthly team meetings such as QA and Safety.
