Job Description
Part time, grant funded opportunity.
Schedule:
Mon, Tue, Wed, Fri 8:00 am – 5:00 pm. In Spring and Summer include one Saturday a month; in Fall and Winter include one Sunday a month. Reliable transportation required between Meriden and Middletown.
The Center for Key Populations (CKP) is a program of the Community Health Center (CHC), Connecticut's largest federally qualified health center, where "health care is a right, not a privilege." Through access to high‑quality primary and specialty healthcare, CKP seeks to remove barriers to comprehensive, respectful, evidence‑based care for key populations in our communities. We collaborate with CHC across the spectrum of health care—including mobile health, quality improvement, training and education, and research—to help transform the lives of the patients we serve.
Roleand Responsibilities
- Apply evidence‑based guidelines from major regulatory and specialty groups; perform pre‑visit preparation for all primary care patients scheduled to be seen.
- Collaborate with the primary care nurse and provider in "e‑huddles" using the Planned Care Dashboard (an electronic tool that summarizes all care due for a patient at the time of their visit) to ensure complete and thorough primary care visits.
- Prepare patients to be seen by provider in the telehealth environment, including guiding patients on the use of video telehealth technology.
- Manage virtual telehealth sessions for multiple providers using Zoom technology, including setup of virtual exam rooms, admitting patients, and pre‑visit preparation.
- Support clinical teams in collecting paperwork, requesting records, scheduling follow‑up visits, and other administrative support tasks for clinical teams.
- Collaborate with providers, clinical and clerical support staff to facilitate appropriate telehealth patient flow.
- Prepare patients to be seen by provider using established criteria as appropriately determined by the nature of the visit (e.g., sick, PE, WCC, etc.).
- Accurately document chief complaint and results from evidence‑based screening tests.
- Accurately perform and document patient vital signs.
- Independently and accurately perform appropriate CLIA waived tests, EKG, hearing & vision tests.
- Prioritize work responsibilities on a daily basis, decreasing waiting time. Collaborate with providers, clinical and clerical support staff to facilitate appropriate patient flow. Effectively work as a member of the health care team, assisting other team members as needed, or requested. Appropriately schedule tests and referrals for specialty care in accordance with provider orders in a timely manner.
- Maintain a clean, orderly, safe working environment; oversee maintenance of supplies and documents in medical logs as directed.
- Set up and stock exam rooms and clean rooms at end of each day worked.
- Adhere to all infection control standards, follow universal precautions and safety policies and procedures of the Agency. Maintain yearly CPR certification as required for all clinical employees.
- Exhibit excellent customer service skills to external and internal customers at all times with a focus on quality of care and teamwork that aligns with the department and Agency mission.
- Comply with Agency mandated in‑service training workshops (EOC, Working Safely, Abuse, etc.).
- High school diploma or equivalency.
- Completion of a Medical Assistant course from an accredited/recognized school.
- Medical Assistant board certification/registration required or to be obtained within 90 days of hire.
- Experience with EKG use, appointment scheduling, word processing, computer skills, and familiarity with medical testing procedures.
- Sound organizational skills and accuracy at all levels of the job.
- Prior demonstrated excellent communication and customer service skills.
- Must be team focused and demonstrate leadership qualities.
- CHC requires current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification. The only acceptable alternative is current American Heart Association BLS/AED for Healthcare Providers certification.
- Bilingual.
- Expert knowledge of the electronic health record.
