Part‑time Social Worker (20 hours)

• Plan, organize, develop, and implement all required social services to meet the needs of residents in accordance with federal, state and local guidelines that govern skilled nursing facilities.
• Perform all documentation requirements for social service documentation in a timely fashion.
• Complete all admission assessments and evaluations.
• Work with the interdisciplinary team to create a plan of care for residents.
• Provide support to residents, families, and staff.
• Attend weekly Care Plan and Medicare meetings.
• Receive and attempt to resolve resident grievances.
• Coordinate resident discharge planning with all relevant departments and assist with scheduling outside services.
• Participate in monthly team meetings such as QA and safety.

• Experience as a social worker within a nursing home or rehabilitation center (strongly preferred).
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
• CT Social Work License (Required).
• Admission experience: 1 year (Preferred).
• Long‑term care experience: 2 years (Preferred).
