Healthcare Nursing, RN Nurse
Pro Health Physicians (CT) is seeking an RN – Family Medicine to join our team in Meriden, CT. This 40‑hour per week position is Monday‑Friday 8:30 am – 5:00 pm, schedule may vary by Practice Manager.
Primary Responsibilities- Order vaccines as needed.
- Oversee the medical team's CLIA competences.
- Serve as liaison between patients, staff and providers.
- Support the care team and its patients by providing and documenting nursing care following established standards and practices.
- Provide patient care and education and assist in locating and coordinating services from other providers and resources.
- Document all patient interactions and other clinical activities accurately and in a timely manner in the electronic health record (EHR).
- Demonstrate respect for patients and staff by presenting a friendly, courteous manner and maintaining patient confidentiality.
- Act as a member of the care team, including collecting and recording basic vital signs and medication information.
- Triage patients according to approved protocols, answer and document patient phone calls, and route messages to providers using the EHR.
- Perform preventive screenings, give injections, and carry out standing orders consistent with Pro Health policies.
- Schedule tests, treatments, and follow‑up visits on behalf of patients, and refer patients when appropriate.
- Monitor task list, follow up on overdue orders, and monitor status of critical referrals.
- Order supplies and vaccines, maintain records, and prepare and maintain equipment, including sterilization.
- Maintain knowledge of current coding, OSHA, CLIA regulations and Pro Health policies.
- Assist the care team to assure smooth office operation and delivery of excellent service through teamwork.
- Conduct outreach to patients in need of appointments or other services, communicate test results, and follow up after appointments to ensure adherence to care plans.
- Perform other duties as assigned.
- Professional education for nursing and Connecticut RN licensure.
- Experience in an acute or primary care setting.
- Experience with immunizations on pediatric patients.
- Basic computer knowledge and aptitude.
- BSN.
- Experience with the AllScript electronic health record.
- Experience with phone triage.
Pay for this role ranges from $28.94 to $51.63 per hour, based on local labor markets, education, work experience, certifications, and other factors.
