Registered Nurse RN Meriden Family ProHealth Physicians Meriden CT

Pro Health Physicians (CT) is seeking an RN – Family Medicine to join our team in Meriden, CT. This 40‑hour per week position is Monday‑Friday 8:30 am – 5:00 pm, schedule may vary by Practice Manager.

• Order vaccines as needed.
• Oversee the medical team's CLIA competences.
• Serve as liaison between patients, staff and providers.
• Support the care team and its patients by providing and documenting nursing care following established standards and practices.
• Provide patient care and education and assist in locating and coordinating services from other providers and resources.
• Document all patient interactions and other clinical activities accurately and in a timely manner in the electronic health record (EHR).
• Demonstrate respect for patients and staff by presenting a friendly, courteous manner and maintaining patient confidentiality.
• Act as a member of the care team, including collecting and recording basic vital signs and medication information.
• Triage patients according to approved protocols, answer and document patient phone calls, and route messages to providers using the EHR.
• Perform preventive screenings, give injections, and carry out standing orders consistent with Pro Health policies.
• Schedule tests, treatments, and follow‑up visits on behalf of patients, and refer patients when appropriate.
• Monitor task list, follow up on overdue orders, and monitor status of critical referrals.
• Order supplies and vaccines, maintain records, and prepare and maintain equipment, including sterilization.
• Maintain knowledge of current coding, OSHA, CLIA regulations and Pro Health policies.
• Assist the care team to assure smooth office operation and delivery of excellent service through teamwork.
• Conduct outreach to patients in need of appointments or other services, communicate test results, and follow up after appointments to ensure adherence to care plans.
• Perform other duties as assigned.

• Professional education for nursing and Connecticut RN licensure.
• Experience in an acute or primary care setting.
• Experience with immunizations on pediatric patients.
• Basic computer knowledge and aptitude.

• BSN.
• Experience with the AllScript electronic health record.
• Experience with phone triage.

Pay for this role ranges from $28.94 to $51.63 per hour, based on local labor markets, education, work experience, certifications, and other factors.
