Pro Health Physicians (CT) – RN – Family Medicine

Pro Health Physicians, part of the Optum family, is seeking a RN – Family Medicine to join our team in Meriden, CT.

As a member of the Optum Care Delivery team, you will help make healthcare better for everyone. You will have clinical resources, data and support from a global organization to help patients live healthier lives.

Pro Health Physicians is a physician‑led medical group with 85 practice locations throughout Connecticut, serving more than 10% of the state’s residents. We empower physicians and advanced clinicians to better serve patients through enhanced care processes that support improved patient outcomes.

Immediate opening for a friendly, patient‑focused, and detail‑oriented RN. 40 hours per week, Monday‑Friday 8:30 am‑5:00 pm. Schedule will be determined by the Practice Manager.

Primary Responsibilities
  • Order vaccines as needed
  • Oversee the medical teams’ CLIA competencies
  • Serve as a liaison between patients, staff, and providers
  • Support the care team by providing and documenting nursing care following established standards and practices
  • Provide patient care, education, and assist in locating and coordinating services from other providers and resources
  • Document all patient interactions and other clinical activities accurately and in a timely manner in the electronic health record (EHR)
  • Demonstrate respect for patients and Pro Health staff by presenting a friendly, courteous manner at all times and maintaining patient confidentiality
  • Provide attention to patients in a manner that builds confidence, trust, and loyalty
  • Act as a member of the care team, including preparing patients to be seen by collecting and recording height, weight, temperature, blood pressure, head circumference, current medications, chief complaint, and history
  • Triage patients according to approved protocols and consistent with Pro Health’s appointment and scheduling guidelines. Answer and document patient phone calls; take complete messages and route to providers as needed using the EHR
  • Provide and document patient care such as giving injections and performing preventive screenings
  • Educate patients regarding their illness or injury and provide instruction regarding self‑management of their condition
  • Carry out standing orders (e.g., authorizing prescription refills and ordering tests) consistent with Pro Health’s policies and procedures
  • Schedule tests, treatments, and follow‑up visits on behalf of patients. Assist patients with identifying suitable outside resources for disease management and other services; refer patients when appropriate
  • Monitor task list to respond to tasks in a timely manner, follow up on overdue orders, and monitor status of critical referrals
  • Order supplies and vaccines and maintain appropriate records. Prepare and maintain equipment, including sterilization
  • Maintain knowledge of current coding, OSHA, and CLIA regulations and Pro Health policies
  • Assist care team to assure smooth office operation and delivery of excellent service through teamwork
  • As appropriate, conduct outreach to patients in need of appointments or other services, to communicate test results and other information, and to follow up after appointments to ensure patients are following their plan of care
  • Perform other duties as assigned
Required Qualifications
  • Professional education for nursing and Connecticut RN licensure
  • Experience in an acute or primary care setting
  • Basic computer knowledge and aptitude
Preferred Qualifications
  • BSN
  • Experience with the AllScript electronic health record
  • Phone triage experience
Pay and Benefits

Pay is based on several factors including local labor markets, education, work experience, and certifications. The hourly pay for this role ranges from $28.94 to $51.63 per hour based on full‑time employment. We comply with all minimum wage laws as applicable. Benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements).
