LPN - Part Time 7a-11am and 3p- 7pm

Aboutthe Role
As an LPN at Meriden Health and Rehab LLC, you will be responsible for providing direct patient care under the supervision of licensed nurses. Your duties will include:
- Administering medications, treatments, and therapies as prescribed by physicians
- Assisting with daily living activities such as bathing, dressing, and grooming
- Monitoring vital signs and reporting any changes to licensed nurses
- Maintaining accurate and up-to-date records of patient care and treatment
- Collaborating with the interdisciplinary care team to develop and implement individualized care plans
- Providing emotional support and comfort to patients and their families
- Provide direct patient care in a skilled nursing facility setting
- Develop and implement individualized care plans in collaboration with the interdisciplinary care team
- Administer medications, treatments, and therapies as prescribed by physicians
- Monitor and report patient vital signs and changes in condition
- Maintain accurate and up-to-date records of patient care and treatment
- Provide emotional support and comfort to patients and their families
- Current LPN licensure in the state of [State]
- High school diploma or equivalent required
- Minimum of 0 years of experience as an LPN
- Ability to lift, move, and transfer patients as needed
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
