Job Description Summary
The Primary Care Nurse (PCN) provides high quality primary nursing care to individuals and families throughout the lifespan. The PCN works at the top of his or her licensure to enhance the safety, effectiveness, and efficiency of care through professional nursing responsibilities and delegation to medical assistants. Provides patients with health education regarding preventive, chronic, and episodic health care. Assumes responsibility with primary care providers to coordinate care, support transitions, and counsel patients.
Applies advanced access, planned care, chronic care model, and rapid cycle improvement at CHCI.
- Support primary care of patients under planned care model and collaborate with PCP to identify preventive and evidence‑based chronic disease care needs.
- Provide general and specific health care education and work with patients to identify and monitor self‑management goals.
- Collaborate with other community‑based providers and follow up on ER visits and hospital discharges.
- Assist providers with exams, treatments, and procedures as needed.
- Ensure safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.
- Administer medications, including vaccines and immunizations, in accordance with provider orders and standing orders as established by CHCI.
- Adhere to CHC protocols for medication documentation, reporting adverse reactions, and providing patient education regarding medications.
- Perform nursing activities such as wound care, catheterization, and other tasks as needed.
- Accurately document all aspects of patient care in the medical chart in a timely manner.
- Schedule tests and referrals for specialty care on the provider’s order.
- Maintain strict infection control standards in accordance with CHCI protocols, OSHA, and CLIA standards.
- Serve as a resource for medical assistants regarding patient preparation, needs, and procedures.
- Monitor and ensure follow‑up of abnormal Pap smear results, STDs, lead screens, and other tests.
- Oversee supplies and equipment maintenance, ordering supplies as necessary.
- Maintain compliance with agency mandatory annual training (Patient Safety, Abuse, Age‑Based Competencies, EOC, Abuse trainings, etc.).
- Use the electronic health record to ensure timely, safe, confidential storage and retrieval of patient information.
Education:
- Bachelor of Science in Nursing (BSN)
- Associate of Science in Nursing (ASN)
