Job SummaryAs a RN MDS, you will be responsible for coordinating the care of our residents, conducting resident assessments, and developing individualized care plans. You will also be involved in the implementation and monitoring of resident care, ensuring that our patients receive the highest level of care and attention.
Responsibilities- Conduct resident assessments and develop individualized care plans
- Implement and monitor resident care, ensuring adherence to facility policies and procedures
- Collaborate with interdisciplinary teams to provide comprehensive care
- Communicate effectively with residents, families, and healthcare professionals
- Participate in quality improvement initiatives to enhance care and patient outcomes
- Maintain accurate and detailed records of resident care and assessments
- Current RN licensure in the state of Connecticut
- 0 years of experience preferred, but we are open to candidates with varying levels of experience
- Strong clinical knowledge and skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
