Staff Nurse - CHC of Meriden

Primary Care Nurse

Schedule:

Monday – Friday 8:00 a.m. – 5:00 p.m., one evening a week 10:00 a.m. – 7:00 p.m., one Saturday a month 8:00 a.m. – 4:00 p.m.

Responsibilities:

• Support primary care of patients under a planned care model.
• Collaborate with PCPs to identify needs for preventive care, evidence‑based chronic disease care, and care coordination.
• Provide general and specific health‑care education and work with patients to identify and monitor self‑management goals.
• Collaborate with community‑based providers and follow up on ER visits and hospital discharges.
• Assist providers with exams, treatments, and procedures as needed.
• Ensure safe, accurate, clinically appropriate collection, storage, and processing of all biologic specimens.
• Administer medications—including vaccines and immunizations—by oral, subcutaneous, intramuscular, intradermal, or nebulizer routes per provider orders and standing orders.
• Adhere to CHC protocols for medication documentation (route, dosage, and patient information).
• Monitor patient response to medications, detect, report, and respond to adverse reactions.
• Provide patient education related to medication administration.
• Perform a wide range of nursing activities as required, including wound care and catheterization.
• Accurately document all aspects of patient care in the medical chart in a timely manner.
• Schedule tests and referrals for specialty care per provider orders.
• Maintain strict infection control standards per CHCI protocols, universal precautions, OSHA, and CLIA standards.
• Serve as a resource for medical assistants regarding patient preparation, needs, and necessary procedures.
• Monitor and ensure follow‑up of abnormal Pap smear results, STDs, lead screens, and other tests performed at CHCI.
• Oversee maintenance of supplies and equipment; order supplies as needed.
• Maintain compliance with mandatory annual agency training (Patient Safety, Abuse, Age‑Based Competencies, EOC, Abuse trainings, etc.).
• Use the electronic health record system expertly to ensure timely, safe, confidential storage and retrieval of patient information.

Qualifications:

• Education: Bachelor of Science in Nursing (BSN) or Associate of Science in Nursing (ASN).
