- Social Services, We are looking for a Community Health Worker to support our clinic.
- Professionally and compassionately serve to connect the community in the Family Medicine Health Center (FMHC) services area to local healthcare and community services and resources.
- Responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.
- Provide social support and informal counseling, advocate for individuals and community health needs, and help to provide services such as glucose test, and blood pressure screening. Community outreach, such as home visits and helping with health screenings at community fairs will be required.

Education:
High School Diploma (required)
Experience:At least 2 years of experience in the medical field.
