Position: Hospice Licensed Practical Nurse (LPN) | Horizon Home Health and Hospice | Meridian, ID

Hospice RN – Meridian, Idaho

Pay Range: $32 - 35, DOE. Job Type: Part-Time. Benefits:

Horizon Home Health & Hospice seeks a compassionate and clinically skilled Hospice RN to join our team in the Meridian, Idaho area.

Responsibilities

Provides direct patient care as defined in the State’s Nurse Practice Act.

Implements plan of care initiated by the registered nurse.

Provides accurate and timely documentation consistent with the plan of care.

Provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.

Participates in coordination of hospice services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the registered nurse and/or Clinical Supervisor or Director of Nursing.

Uses equipment and supplies effectively and efficiently.

Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct.

Participates in personal and professional growth and development.

Performs other duties as assigned by the registered nurse.

Physical Requirements & Work Environment

Prolonged or considerable walking or standing.
Ability to lift, position, and/or transfer patients.
Ability to lift supplies and equipment.
Considerable reaching, stooping, bending, kneeling, and/or crouching.
Visual acuity and hearing to perform required nursing skills.

Qualifications

LPN licensed in Idaho.
Valid Driver’s License and Auto Insurance.
Hospice experience preferred.
