Responsibilities
  • Complete initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
  • Provide complete physical assessment and history of current and previous illnesses.
  • Utilize all elements of the nursing process to provide professional nursing care.
  • Write and initiate plans of care; re‑evaluate patients and families regularly.
  • Participate in revising plans of care as necessary.
  • Use health assessment data to determine nursing diagnoses.
  • Develop care plans with patient and family input, incorporating palliative nursing actions.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed.
  • Counsel patients and families on nursing and related needs.
  • Provide health care instructions per assessment and plan.
  • Assist patients and caregivers with activities of daily living, education, and maintain independence or comfort care as appropriate.
  • Adhere to organization documentation, care procedures, and professional conduct standards.
  • Act as case manager when assigned, coordinating patient care for assigned caseload.
  • Maintain accurate clinical notes; record pain/symptom management changes and outcomes.
  • Communicate with physicians regarding patient needs and report changes; obtain physician orders as required.
  • Coordinate with community health providers to align care plans.
  • Teach self‑care techniques to patients and families.
  • Provide medication, diet, and other instructions as ordered; recognize health counseling opportunities.
  • Collaborate with interdisciplinary group; attend meetings.
  • Maintain a safe environment for patients.
  • Assist families and team members in continuity of care.
  • Work in cooperation with hospice interdisciplinary group to address emotional needs.
  • Participate in on‑call duties per policy; ensure equipment and services are available.
  • Supervise ancillary personnel and delegate responsibilities when required.
  • Maintain professional growth; attend continuing education and in‑service classes.
  • Fulfill case assignments and participate in quality assessment and performance improvement activities.
Qualifications
  • Registered Nurse with current licensure to practice professional nursing in Idaho.
  • Graduate of a National League for Nursing accredited school.
  • Valid CPR certification.
  • Licensed driver with insured, operating automobile in good working order.
  • Minimum 2 years experience in home care, hospice, or public health nursing (preferred).
  • Strong clinical skills and compassionate bedside manner.
  • Independent, self‑directed, able to work with minimal supervision.
  • Excellent observation, problem‑solving, verbal and written communication skills.
  • Ability to organize and prioritize workload independently.
  • Experience supervising hospice aides (preferred).
Physical Requirements & Work Environment

• Prolonged or considerable walking or standing.

• Ability to lift, position, and/or transfer patients; lift supplies and equipment.

• Reaching, stooping, bending, kneeling, and/or crouching.

• Adequate visual and hearing acuity to perform nursing skills.
