Job Description Summary

An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home or in an alternate infusion suite, ensuring patient safety and continuity of care under a physician’s plan of care. The role is performed under the structured supervision of a designated supervisor.

Job Responsibilities
  • Initiates, develops, and implements intermediate nursing plans of care treatments, evaluating patient progress toward goals.
  • Organizes and participates in direct patient care, including treatments, medication administration, and patient/family education.
  • Modifies plans of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals.
  • Assesses patient needs and physical status at each skilled visit through health data access and patient interview.
  • Re‐evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders.
  • Obtains and clarifies physician orders for plan of treatment revisions, informs physicians promptly of significant changes, and provides written summaries.
  • Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
  • Teaches patients and families using a patient‑centered approach to achieve independence with prescribed therapy through active participation.
  • Communicates effectively and timely with the clinical team, medical providers, patients, and families to facilitate continuity of care.
  • Prepares clinical documentation in real‑time during visits and submits it to the nursing department to comply with billing timelines.
  • Coordinates discharge planning and prepares discharge summaries with patient instructions, reporting patient care needs, progress, and goals when transferring care.
  • Demonstrates compliance with agency operations, policies, professional standards, and regulatory guidelines.
  • Maximizes work efficiency through the use of computers and automation technologies to validate orders and manage visit schedules.
  • Responds promptly to patient requests and initiates emergency procedures as necessary.
  • Accepts accountability for own practice through ethical and professional conduct.
  • Ensures safeguarding of patient confidentiality and proprietary information in accordance with HIPAA and other regulations.
  • Provides oversight and coordination of paraprofessionals in the home setting and may act as supervisor if required.
  • Participates in multidisciplinary team conferences, precepting, training, and mentoring other nurses for orientation and supervisory activities.
  • Participates in nursing department on‑call responsibilities and performs other related duties as directed.
  • Perform PICC/Midline insertions per physician order, troubleshoot/declot obstructed central lines, monitor PICC/Midline, and educate on prevention of line infection.
Basic

Education and/or Experience
  • Active and unrestricted RN license in the state of practice.
  • Minimum of one year of nursing experience.
  • Current CPR certification required.
Basic Qualifications
  • Demonstrated competency in patient care standards for safe delivery of infusion services.
  • Advanced certification and training as applicable.
  • Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel and proof of liability insurance.
  • Basic knowledge of computer operating systems and software applications with the ability to use nursing technology tools for communication and documentation.
Physical Demand Requirements

Ability to lift up to 50 pounds, with a maximum lifting of 75 pounds. Exerting up to 20 pounds of force occasionally, or up to 10 pounds frequently. Physical demands include walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting, and carrying; fine motor skills and visual acuity are required.
