Position OverviewAs a Hospice Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within Boise, surrounding regions to deliver in‑home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.
WhatYou Can Expect In This Role
- Full‑time
Schedule:
Monday to Friday, 8:30 AM – 5:00 PM, with rotating evening on‑call and weekend shifts - Patient Population:
Adults – interest in training for an occasional pediatric patient - Manageable Caseloads:
Daily visit requirements are below the state and national average - Compensation:
Paid hourly, which includes drive time and mileage reimbursement for traveling within Boise and its surrounding regions - Additional Pay:
On‑call pay and applicable differential pay - Bonuses:
Sign‑on bonus and relocation assistance available (where applicable)
- Tuition Reimbursement:
Up to $5,250 per calendar year for degree advancement - Certification Reimbursement:
Support for certifications above and beyond your role requirements, with extra pay for eligible certifications - Additional Training:

Qualifications- Education:
ASN or BSN in Nursing - Experience:
2 years of RN experience, Hospice Case Management experience preferred - Licenses/Certifications:
Current RN license in the state of Idaho;
Basic Life Support (BLS) certification - Other Requirements:
Valid driver’s license and proof of auto insurance
