Position Overview

Job Responsibilities
  • Complete an initial assessment of patient and family to determine home care needs and provide a comprehensive assessment and medical history.
  • Regularly re‑evaluate patient nursing needs.
  • Initiate and update the plan of care as patient status changes.
  • Use assessment data to determine nursing diagnoses.
  • Develop a care plan, establish goals, and incorporate therapeutic, preventive, and rehabilitative nursing actions with patient and family participation.
  • Initiate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed by the physician.
  • Counsel patient and family to meet nursing and related needs.
  • Provide health‑care instructions appropriate to the assessment and plan of care.
  • Identify discharge planning needs and implement them prior to patient discharge.
  • Act as case manager when assigned and coordinate care for the assigned caseload.
  • Adhere to organizational documentation and care procedures and uphold professional conduct standards.
  • Prepare clinical notes and update the primary physician at least every sixty days.
  • Communicate with the physician regarding patient needs and report any changes in condition; obtain physician orders as required.
  • Communicate with community health personnel to coordinate the care plan.
  • Participate in on‑call duties as defined by the on‑call policy.
  • Ensure arrangements for equipment and other necessary items and services are available.
Qualifications
  • Registered Nurse (RN) licensed in Idaho.
  • Valid driver’s license and auto insurance.
  • Experience in home health and/or hospice preferred.
