Position: LPN - Home Health & Hospice - Full Time - Up to $35/hr!

Licensed Practical Nurse

Licensed Practical Nurse needed for full-time home health and hospice care near Meridian, Idaho. Provide skilled nursing and compassionate support to patients in their homes, ensuring comfort and dignity. Join a dedicated team delivering quality end-of-life and home health services.

Key Responsibilities:
  • Provide skilled nursing care to home health and hospice patients in accordance with physician orders and individualized care plans
  • Monitor and document patient conditions, symptoms, and responses to treatment
  • Administer prescribed medications and treatments safely and accurately
  • Assist patients with disease management, symptom control, and comfort measures
  • Educate patients and family members on medications, treatments, and ongoing care needs
  • Collaborate with physicians, registered nurses, therapists, social workers, and other members of the interdisciplinary care team
  • Observe and report changes in patient status promptly
  • Maintain accurate and timely electronic medical documentation
  • Promote patient safety while maintaining compliance with state and federal regulations
  • Participate in care conferences and contribute to individualized patient care planning
  • Deliver compassionate emotional support to patients and their families throughout the continuum of care
Qualifications:
  • Current Licensed Practical Nurse license in the State of Idaho or eligible for Idaho licensure
  • Graduate of an accredited Practical Nursing program
  • Minimum of 1 year of Home Health and Hospice nursing experience
  • Current BLS (Basic Life Support) certification
  • Valid driver's license with reliable transportation and current auto insurance
  • Ability to travel throughout assigned service areas
  • Strong assessment, communication, and organizational skills

  • Compassionate bedside manner with strong interpersonal communication
  • Ability to work independently while collaborating effectively with an interdisciplinary team
  • Knowledge of home health regulations and hospice philosophy of care
