United Health Group, through Northern Arizona Hospice, part of the Optum family, seeks a Registered Nurse Case Manager to deliver the hospice Plan of Care and collaborate with the IDG to ensure quality, compassionate care for patients and families.
You will evaluate performance, implement strategies based on nursing theory, and promote physical, psychosocial, emotional, and spiritual well-being in patients and families. This is a full-time role with comprehensive benefits.
