Position: LPN - Home Health & Hospice - Full Time - Up to $35/hr!

Licensed Practical Nurse works full-time in home health and hospice, delivering skilled nursing care to patients in their homes. The role supports comfort, dignity, and quality of life while collaborating with an interdisciplinary team.

Key Responsibilities:
  • Provide skilled nursing care to home health and hospice patients in accordance with physician orders and individualized care plans

  • Monitor and document patient conditions, symptoms, and responses to treatment

  • Administer prescribed medications and treatments safely and accurately

  • Assist patients with disease management, symptom control, and comfort measures

  • Educate patients and family members on medications, treatments, and ongoing care needs

  • Collaborate with physicians, registered nurses, therapists, social workers, and other members of the interdisciplinary care team

  • Observe and report changes in patient status promptly

  • Maintain accurate and timely electronic medical documentation

  • Promote patient safety while maintaining compliance with state and federal regulations

  • Participate in care conferences and contribute to individualized patient care planning

  • Deliver compassionate emotional support to patients and their families throughout the continuum of care

Qualifications:
  • Current Licensed Practical Nurse (LPN) license in the State of Idaho or eligible for Idaho licensure

  • Graduate of an accredited Practical Nursing program

  • Minimum of 1 year of Home Health and Hospice nursing experience

  • Current BLS (Basic Life Support) certification

  • Valid driver's license with reliable transportation and current auto insurance

  • Ability to travel throughout assigned service areas

  • Strong assessment, communication, and organizational skills

Preferred

Skills:
  • Excellent time management and independent decision-making skills

  • Compassionate bedside manner with strong interpersonal communication

  • Ability to work independently while collaborating effectively with an interdisciplinary team

  • Knowledge of home health regulations and hospice philosophy of care

  • Commitment to delivering exceptional patient-centered care

  • Experience with electronic medical records

You hold a current Idaho LPN license. You have at least one year home health hospice experience. You possess strong assessment, communication, and time-management skills
