Position: Registered Nurse(RN) Case Manager - Hospice - Full Time - Up to $100,000/YR!

Registered Nurse Case Manager in hospice provides skilled nursing care to patients in home and community settings, coordinating interdisciplinary teams to support patients and families through end-of-life care. The role involves comprehensive assessments, individualized care planning, and symptom management to ensure comfort and dignity.

Key Responsibilities:
  • Provide compassionate nursing care and symptom management to hospice patients in home and community settings
  • Serve as primary case manager for assigned patient caseload
  • Perform comprehensive nursing assessments of physical, emotional, psychosocial, and spiritual needs
  • Develop, implement, and regularly update individualized hospice care plans
  • Coordinate patient care with physicians, aides, social workers, chaplains, volunteers, and other team members
  • Monitor changes in patient condition and adjust care plans accordingly
  • Communicate significant status changes to attending physicians and the hospice medical director
  • Provide education and emotional support regarding disease progression, symptom management, and end-of-life care
  • Administer medications and treatments in accordance with orders and individualized plans
  • Evaluate intervention effectiveness and recommend adjustments to care
  • Maintain accurate, timely, and thorough clinical documentation per regulations
  • Manage a field-based schedule efficiently while traveling between patient visits
Qualifications:
  • Current and unrestricted Registered Nurse (RN) license in Idaho or eligibility for Idaho RN licensure
  • Associate Degree in Nursing (ADN) from an accredited program;
    Bachelor of Science in Nursing (BSN) preferred
  • Current BLS/CPR certification for healthcare providers
  • Valid driver's license and reliable transportation for home and community visits
  • Ability to meet applicable automobile insurance requirements
  • Strong clinical assessment, documentation, communication, and organizational skills
Preferred

Skills:
  • Previous experience as a Hospice RN Case Manager
  • Hospice or palliative care nursing experience
  • Home health nursing experience
  • Familiarity with Medicare hospice regulations and requirements
  • Knowledge of pain management and symptom control
  • Experience working with interdisciplinary healthcare teams
  • Strong understanding of end-of-life care and family support
  • Ability to work independently while maintaining effective communication with the hospice team
  • Compassionate, patient-centered approach to care
  • Strong time-management and case-management skills

You hold an unrestricted Idaho RN license and BLS certification. You have hospice nursing experience and can manage independent caseloads. You excel at interdisciplinary coordination and compassionate end-of-life care
