OverviewTo provide information, referrals, or assessment of consumer needs that link people with programs or services in an effort to maximize personal independence. Options Counselors are responsible for providing accurate information regarding programs that service the elderly, caregivers, and vulnerable adults in the Southwest Idaho area. These programs include, but are not limited to, support services, community-based in‑home services and access services.
Duties- Ability to quickly learn about Area Agency on Aging programs, eligibility, and service authorization.
- Conducts intake for Adult Protective Services reports over the phone or through the Idaho Commission on Aging Online Portal. Provide follow‑up as necessary or directed.
- Performs intake, assessment and authorization for Home Delivered Meals, Homemaker, and Respite Services using approved agency database.
- Performs monthly follow up of Respite caseload and quarterly follow up of Homemaker caseload.
- Provides assistance and referrals over the phone, in the office, or in the community setting. Participation in outreach events as assigned.
- Ensure incoming calls are prioritized and answered in a timely and accurate fashion.
- Input accurate documentation that is concise, thorough and with minimal grammatical errors.
- Coordinate and monitor formal and informal service delivery including management of service waitlists.
- Schedules and conduct follow‑ups to ensure authorized services are received, unmet needs are met and/or determine continued eligibility and/or terminate services.
- Performs other related duties as may be assigned.
Skilled in common Microsoft Office programs and data entry. Prior call center or remote customer service experience is a big plus.
EducationHigh school diploma required, AA/BA/BS ideal.
