Nurse Practitioner seeking to deliver comprehensive primary care services across a variety of settings—including in-home visits, assisted living and nursing facilities, post-acute care hospitals, and via telehealth.

• Provide primary medical care services in home, facility, and telehealth settings
• Conduct physical exams, wellness visits, and manage chronic and acute conditions

• DEA license required
• Experience in home health, hospice, or facility-based care required
• Strong communication, decision-making, and clinical skills
