Role
Registered Nurse (RN) Home Health Case Manager, responsible for delivering personalized patient care in clients' homes.
Key DutiesManage caseloads, supervise LPN visits, conduct assessments, develop and implement care plans based on physician orders, and collaborate with healthcare professionals including Physical, Speech, and Occupational Therapists. Communicate with health insurance companies and provide education on conditions such as CHF, COPD, Diabetes, and Wound Care. Use AI tools for efficient charting to promote work-life balance.
Qualifications & SkillsValid Kansas or Compact RN license, Driver's license, and vehicle with insurance. Home health or critical care experience preferred. Strong physical assessment, diagnostic, and communication skills essential.
