Position SummaryThe Integrated Health Services Nurse Care Coordinators work in our START with HOPE Program. Our nurses’ primary role is to ensure that every child receives the direct and proper care they need. Nurses assess and identify patients’ needs, then implement and monitor the patient’s medical plan and treatment. They also ensure that patient care is conducted according to the policies and standards of the department.
Nurses coordinate the care for every patient. From drawing blood and educating patients on their health to working closely with Doctors, Nurse Practitioners or Physician Assistants. Nurses implement prescribers’ instructions.
- Registered Nurse (RN) license required.
- Spanish Speaking Preferred
- BSN preferred.
- Current CPR certification
- Courses in Child Abuse Identification and Infection Control.
- 1+ years of related work experience with children and/or families
- Excellent communication skills (verbal and written)
- Proficiency in Microsoft Word and Outlook
- Ability to engage with clients
- Understanding and willingness to commit to the agency’s Mission, core values, Sanctuary Commitments and social justice.
- Conduct Physical Health Assessments for Infants, Children, Adolescents, including HIV Risk Assessments, Safety Assessments, General First Aid and Triage Activities.
- Perform medical orders (tests, tasks, hospitalizations, etc.) as ordered by physician, nurse practitioners and/or physician assistants.
- Ensure required documentation for all case actions (medical notes and medication administration documentation) are completed within established time frames.
- Maintain appropriate professional boundaries with clients and staff.
- Maintain a safe and secure medical environment for clients.
- Take full vital signs, including but not limited to, heights, and weights; document observations and activities, and act upon and/or report abnormal findings to the provider.
- Prepare and administer immunizations.
- Provide first aid and emergency care as needed.
- Complete diagnostic tests such as phlebotomy, hearing test, vision test and EKGs among others.
RN: $40.00 - $44.00/hour – Part‑Time
1‑2 days/week / 4‑8 hours/week M‑F (no weekends)
Hiring Incentive – $1,000 after 500 worked hours.
