Premise Health Inc. is seeking a Part-Time Nurse Practitioner to provide care to client employees and their dependents at our Health Center in Merrillville, IN. The role supports a patient-focused healthcare model with on-site clinic services.
Schedule:
29 hours per week, with one late evening per week;
Tue, Thu, Fri 9:00 am–5:00 pm;
Mon/Wed 9:00 am–7:00 pm; annual performance bonus eligibility. You’ll perform exams, treatments, care plans, and education, collaborating with physicians and teams.
