Case Management Social Worker

Sevita is seeking a Case Management Social Worker to coordinate person-centered services for individuals with complex behavioral and medical needs. In this role, you will complete psychosocial assessments, develop and monitor care plans, connect individuals and families with community resources, and advocate for needed services. You'll collaborate closely with clinical and community teams, document in compliance with regulations, and support crisis intervention when needed.

Responsibilities

• Conduct comprehensive psychosocial and needs assessments for individuals and families.
• Develop, implement, and regularly update individualized, person-centered care plans.
• Coordinate services across healthcare, behavioral health, and community providers.
• Link clients and families with appropriate community resources and supports.
• Advocate for client needs and ensure timely access to services.
• Monitor progress, adjust plans, and document all activities in EHR systems.
• Collaborate with interdisciplinary teams to support holistic care.
• Assist with crisis intervention and safety planning as needed.
• Ensure compliance with regulatory, payer, and organizational documentation standards.
• Participate in ongoing training and professional development offered by Sevita.
