• Develop interventions to support therapeutic goals and restore maximum function and strength, promoting optimal health and minimal pain.
• Evaluate patients’ progress by observing, noting, and recommending adjustments as needed.
• Complete patient admission to the agency’s services after thorough review of medical records.
• Assure continuation of the therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.

• Bachelor’s degree in Physical Therapy.
• At least two years of therapy experience in a hospital or clinic setting.

• Master’s or Doctorate degree in Physical Therapy.
• Home‑care experience.

• Registered as a Physical Therapist with New Hampshire’s Board of Nursing.
• CPR Certification.
• Valid criminal record check.
• Valid motor vehicle record.
