About the Job

As a Licensed Nursing Assistant
, you’ll play a key role in delivering compassionate, high-quality personal care to patients in their homes. You’ll work under the direction of an RN and collaborate closely with the care team to support patient comfort, safety, and well-being. This role is hands‑on, relationship‑driven, and essential to helping patients remain safely and comfortably where they want to be—at home.

What You’ll Do
  • Provide personal care services including bathing, dressing, grooming, toileting, and safe transfers.
  • Support household needs such as light housekeeping, laundry, and meal preparation (after completing personal care tasks).
  • Use mechanical devices and proper body mechanics to ensure safe mobility assistance.
  • Observe and report changes in physical or mental condition, intake/output, and overall patient status.
  • Cue patients for medications as directed in the care plan.
  • Maintain a clean, safe, and healthy environment following aseptic and safety standards.
  • Communicate professionally with patients, families, and the care team.
  • Complete timely and accurate documentation.
  • Uphold professional boundaries and represent the agency with excellent customer service.
  • Travel within the agency’s service area to provide care.
Who You Are
  • A Licensed Nursing Assistant in good standing with the New Hampshire Board of Nursing.
  • CPR certified, with a satisfactory background check and motor vehicle record.
  • Reliable, compassionate, and committed to delivering exceptional patient-centered care.
  • Comfortable working independently in the home setting while collaborating with a supportive clinical team.
  • Preferred: experience in home care and/or caring for patients with dementia, Alzheimer’s disease, COPD, cancer, or post‑surgical needs; ability to perform PT/INR checks.
