In-Home Health - Nurse Practitioner or Physician Assistant; Part Time - Merrimack County, NH

Position Summary

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. You will engage with people in the comfort of their own homes and help bridge gaps in care.

• Conduct a 45‑minute visit that includes a medication and medical history review, a physical examination, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) as ordered by the patient’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; you are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
• Bring your heart into every visit, building trusted relationships and connecting patients to the right care for their unique needs.
• Travel locally and, as needed, within licensed states.
• Conduct virtual visits when required.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products such as spirometry, DEE, and bone density screening.

Your primary workplace is the private residence of health plan members, which may contain pets, secondhand smoke, and other allergens or irritants. You must be able to perform duties safely in these settings.

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required); multi‑state licenses (preferred).
• Ability to cover the following primary locations:
  Merrimack County.

Part‑time: 29.5 hours per week. Base hourly rate, $41.85–$90.13. Pay may vary by experience, education, geography and other criteria. Additional reimbursement opportunities include pay per completed Diagnostic and Preventive Services.
