Southern New Hampshire Cardiology Nurse Practitioner Job – 1 Hour from Manchester Permanent New Hampshire

- Permanent job opening
- Full-time position
- Must have one or more years of post-training experience
- Cardiology, internal medicine, primary care, or related specialty experience preferred
- Work collaboratively with cardiologists, physicians, and multidisciplinary healthcare teams
- Opportunity to practice at the top of your license while caring for patients with acute and chronic cardiac conditions
- Perform comprehensive cardiology assessments, including patient histories and physical examinations
- Evaluate, diagnose, and manage a wide range of cardiovascular conditions
- Order and interpret laboratory, diagnostic, and imaging studies
- Develop individualized, evidence-based treatment plans
- Prescribe medications and therapies in accordance with state regulations and clinical guidelines
- Monitor patients between visits and ensure continuity of care
- Triage patient calls and provide clinical consultation
- Educate patients and families on heart disease prevention, treatment plans, lifestyle modifications, and wellness strategies
- Participate in community outreach and heart health education initiatives
- Maintain accurate, timely, and compliant clinical documentation
- Support quality improvement initiatives and a culture of patient-centered care

Required Qualifications:
- Graduate of an accredited School of Nursing
- Successful completion of an accredited Nurse Practitioner program
- Current New Hampshire APRN license
- Current DEA registration
- Ability to obtain and maintain professional liability insurability through the organization
- Strong clinical assessment and diagnostic skills
- Ability to interpret diagnostic testing and formulate treatment plans
- Knowledge of evidence-based cardiology practices
- Excellent communication, patient education, organizational, and time-management skills
- Ability to prioritize, problem solve, work independently, and collaborate effectively
