Job Summary

The Transition Facilitator develops and implements transition programs focused on the social, intellectual, and vocational growth of youth with disabilities at Mesa Public Schools. This position coordinates career counseling and career development services for youth in junior high and high schools to ensure a seamless transition from school to the adult world. This position adheres to the teacher salary placement and teacher calendar.

Candidates must complete an on-line application which requires appropriate Arizona certification, a valid IVP Arizona fingerprint clearance card, evidence of highly qualified status, three letters of recommendation, and transcript of credits.

Key Responsibilities
  • Serve as interagency liaison between the youth, the family, the school, community and state agencies, and employers
  • Coordinate, administer and/or interpret career development and assessment tools, working with the youth, family, special educator, career and technical educator, and counselor when appropriate
  • Coordinate program development at the junior and high school levels which support transition planning and provide a continuum of career development and life planning activities
  • Function as a resource for classroom teachers in providing mini-units and/or co-teaching opportunities of employability skills, self-advocacy skills, and independent living skills
  • Provide coordination and reporting of vocational placement and follow-up for Mesa Public Schools
  • Facilitate IEP meetings at the junior and high schools related to transition service needs of students
  • Assist youth in overcoming barriers to job search activities and in obtaining employment
  • Inform parents and families of community resources to facilitate access and adult agency service coordination
  • Valid Arizona driver’s license; must be van certified or ability to obtain van certification
  • Assist with other assigned duties as appropriate to the area of assignment and Mesa Public Schools
Required Qualifications
  • Bachelor’s degree in vocational evaluation, counseling, special education, psychology, transition, or related field
  • Master’s degree (Preferred) in vocational evaluation, counseling, special education, psychology, transition, or related field
  • Valid Arizona Special Education teaching certificate
  • Five years experience in special education, vocational training, or related area providing services to youth with disabilities
  • Experience in aligning special education and general education systems (curriculum, assessment, etc.) for student-driven transition outcomes
