Case Manager, Patient Services -Rare Disease; West Coast

• Provide personalized assistance to patients and caregivers in accessing PTC products
• Build positive relationships with patients, caregivers, healthcare providers, and internal teams
• Solve urgent problems to ensure patients gain access to therapy and maintain adherence
• Maintain confidentiality of patient health information while coordinating care
• Navigate the CRM system and ensure accurate documentation of patient cases

• Bachelor’s degree with 3 to 5 years of Case Management experience or equivalent education and experience in the orphan/rare disease biotech environment
• Strong problem-solving and solution-oriented skills
• Ability to work independently and collaboratively in a team setting
• Proficiency in Microsoft Office
• Excellent verbal and written communication skills

• Bilingual/Spanish-speaking skills
• Experience as a Pharmacy Technician
• Prior experience in HCP Office Management
• Familiarity with working alongside Sales Representatives
• Registered Dietitian License (preferred but not required)
