Advanced Home Health and Hospice of Arizona is seeking a compassionate and clinically skilled Physical Therapist to join our team. The Physical Therapist provides skilled, patient-centered care in the home setting and uses clinical expertise to support mobility, safety, independence, and quality of life. This role is ideal for a licensed professional who values meaningful patient relationships, clinical autonomy, and collaboration with an interdisciplinary care team.
Full-time position
Monday-Friday
Weekday schedule

Have a desire to work with an organization committed to compassionate, high-quality care
Current Physical Therapist license in the State of Arizona
Graduate of an accredited Physical Therapy program
Current CPR certification
Valid driver's license, reliable transportation, and automobile insurance that meet agency requirements
Strong clinical judgment, organization, and communication skills
Ability to work independently while collaborating effectively with an interdisciplinary team
Home health experience is preferred

Skilled at communicating with patients, families, and members of the care team
