Position: Home Health Physical Therapy (PT)

Physical Therapist

Advanced Home Health and Hospice of Arizona is seeking a compassionate and clinically skilled Physical Therapist to join our team. This position offers the opportunity to provide high-quality, patient-centered care in the home setting while helping patients improve mobility, strength, safety, and independence.

The Physical Therapist provides skilled, patient-centered care in the home setting and uses clinical expertise to support mobility, safety, independence, and quality of life. This role is ideal for a licensed professional who values meaningful patient relationships, clinical autonomy, and collaboration with an interdisciplinary care team.

Full-time position

Monday-Friday

Weekday schedule

Health, dental, and vision insurance for employees and eligible dependents, beginning the first day of the month following 60 days of employment

Paid time off and holiday benefits in accordance with company policy

$25,000 of company-provided life insurance, with additional coverage options available

401(k) participation after 60 days of employment, including a company match of 25% on the first 4% contributed

Building trusted relationships with patients, families, and caregivers

Collaborating with an interdisciplinary care team

Enjoy a consistent weekday schedule, Monday through Friday

Have a desire to work with an organization committed to compassionate, high-quality care

Current Physical Therapist license in the State of Arizona

Graduate of an accredited Physical Therapy program

Current CPR certification

Valid driver's license, reliable transportation, and automobile insurance that meet agency requirements

Strong clinical judgment, organization, and communication skills

Ability to work independently while collaborating effectively with an interdisciplinary team

Home health experience is preferred

Organized and dependable

Comfortable providing care independently in patients' homes

Skilled at communicating with patients, families, and members of the care team

Committed to helping patients achieve meaningful functional goals
