Occupational Therapist, Physical Therapist, Speech Therapist

• Evaluate patients' functional abilities and limitations
• Determine whether intervention is needed
• Establish plans of care and measurable treatment goals
• Complete comprehensive evaluations within departmental time frames
• Implement individualized physical therapy treatment plans
• Provide skilled treatment safely and assess ongoing effectiveness
• Modify plans of care as indicated
• Involve patients and families/caregivers in treatment and decision-making
• Provide education to physicians, nurses, patients, families/caregivers, and interdisciplinary teams
• Document evaluations, treatment, progress, and reassessments according to professional practice guidelines
• Communicate patient information, treatment results, and progress to physicians, nurses, patients, families/caregivers, and interdisciplinary teams
• Contact physicians for problem solving when needed
• Adapt assignments to caseload, scheduling, and staffing needs
• Supervise and delegate to PTAs, ATCs, Therapy Technicians, and students
• Participate in training, orientation, meetings, and departmental process improvement
• Meet departmental productivity standards
• Interact with staff, physicians, patients, families, external health care institutions, community providers, agencies, and physician offices

• Graduated from an APTA accredited school of Physical Therapy, or equivalent
• Current Physical Therapy license within the State of practice
• BLS certification required
• Ability to become proficient with computer software applications commonly used in an automated patient care environment
• Ability to use patient care equipment, evaluation tools, and therapeutic modalities
• Arizona Fingerprint Clearance Card required at the time of hire and maintained throughout employment for employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility
• Additional related education and/or experience preferred

Demonstrates expertise in evaluating patients' functional abilities, establishing individualized treatment plans, and providing skilled physical therapy interventions. Proficient in documentation, communication with interdisciplinary teams, and adapting care based on patient needs and departmental standards.
