Housing Specialist

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness. Job Summary

The F/ACT Housing Specialist works with a multidisciplinary team to implement the permanent supportive housing model with members assigned to their respective F/ACT team. The F/ACT Housing Specialist is expected to work with community supportive housing agencies on temporary and permanent housing opportunities for their clients. The F/ACT Housing Specialist will provide in field support to clients and work with landlords to ensure ongoing safety to the client as well as the appropriateness of the placement.

Skills/Requirements
  • High school Diploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.
  • 1-3 years of experience in behavioral health working in multiple settings and levels of care, including both mental health and substance abuse preferred.
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred.
  • Current AZ Driver's License in good standing.
  • Clear 39-month Motor Vehicle Record.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R and CBI Clinical Policies and Procedures.
  • Fingerprint Clearance Card required.
  • SOAR certification preferred.
  • Certified to complete the Vulnerability Index Service Prioritization Decision Assistance Tool (VI-SPDAT) preferred.
